REGISTRATION DATES:

Wendesday March 14th from 6 pm to 8 pm.
Saturday March 17th from 10 am to 1 pm.
Saturday April 14th from 10 am to 1 pm.

** We will have a late registration date of Wendesday April 25th from 6 pm to 8 pm. There will be an additional charge of $25 for this sign up.

REGISTRATION LOCATION: Willow Creek Middle School ( Caffeteria)

WHAT TO BRING TO REGISTRATION:

  1. Your full registration fee ( Football or Cheer $245) Cash/Check/Money order No credit/debit cards can be taken.
  2. Your child's original birth certificate.
  3. Your 2012 registration forms (registration/player contract and parent contract) These forms can be printed from the link below. 4) A Physical filled out on the Pop Warner form (print from below). We will have either a nurse practionier or a doctor at the first 3 registrations to perform physicals if you require it for a $25 fee.

As in the past years,if your child is a football player, he/she will be required to be there to try on a jersey and be weighed. The jersey numbering selection will be done on a first come basis.

We will not be taking requests for players to be assigned to a specific coach. (There are no coaches selected as of yet!) You can however request your child be put on the same team as another child to ease your burden of getting your child to practice, games, etc.. However, please keep in mind that we will do our best but will not guarantee that it will happen.

Once your child is assigned to a team, there will be no changing teams for any reason other than a weight issue.

If you would like to volunteer as a coach, please fill out the attached volunteer form and bring it to one of the registrations along with a copy of your drivers license.
